Ok at Fanime the was a Video of Safety at the work place, about a guy that drives a forklift. Needless to say he dies and many other workers in the plant do too. This video was Live action. Probably 5 min long?

That is from Anime Hell.. I am trying to think of the name of the video but all i can remember that it was in German and cheesy ultra violent.
