Hints seem to indicate that it'll be based on book 4: Suzumiya Haruhi no Shoushitsu/The Disappearance of Suzumiya Haruhi.

No airing date as far as I can find.

I don't recall, nor can I find any official word on a quick Google.  As far as I can tell, that was all speculation.
