I usually go in the office and they fill it out for me so I am kinda confused when filling out the papers (I live out of state.)
If someone can be so awesome to help and tell me what should go where in this section I would be so grateful!

I filled mine out at work sooo here goes...

36.Retail (most likely) 37. one 38. Art (and whatever else you'll be selling) 39. (up to you) 40. your address

and 41 through 49 is usually your address and landlord information unless you own a building where you do business elsewhere

50-51 are just estimations, what you think you'll be making at con

53-55 are most likely going to be no

What Elimeno said is correct, but you can usually simply leave the landlord information blank. Since you're just selling at an event, they don't really expect  you to know who the actual landlord for the convention center is.
